Thank you for your purchase of a paver brick at the Police Memorial.

Your willingness to help us “Keep The Promise” to never forget Northeast Ohio Police Officers who have paid the ultimate sacrifice in order to serve and protect us.

Please note that pavers must be purchased and all information provided by April 1 of each year to be installed by May for the Police Memorial Comemmoration.

You MUST complete this form and mail it to the Greater Cleveland Peace Officers Memorial Society after paying via PayPal for the Paver.
